Services
At Real Community Services (RCS), we understand the importance of specialised clinical and allied health support in delivering high-quality disability services. We work in collaboration with your chosen health professionals, including nurses, occupational therapists, physiotherapists, speech pathologists, and other allied health specialists, to ensure that support workers are equipped with the skills and knowledge required to provide safe and effective care.
Through a coordinated approach, we support participants in accessing the clinical care and therapies they need while ensuring staff receive training aligned with the NDIS High-Intensity Support Skill Descriptors.

Clinical & Nursing Support

Comprehensive Health Assessments & Care Planning – Individualised evaluations to support participant health and well-being.

Wound Care Management – Assistance in following clinical guidelines for wound care.

Medication Administration & Management – Supporting safe medication administration under the guidance of health professionals.

Chronic Disease Management – Ensuring support aligns with health professionals’ recommendations for conditions such as diabetes, hypertension, and respiratory illnesses.

Epilepsy & Seizure Management – Implementing individualised seizure care plans and emergency response strategies.

Gastrostomy (PEG) & Bowel Management – Ensuring personal care aligns with participant health plans.

Catheter & Stoma Care – Supporting hygiene and care needs as directed by clinical specialists.
Therapy & Allied Health Support

Speech Pathology & Communication Support – Ensuring support workers are trained in communication strategies, dysphagia and Augmentative and Alternative Communication (AAC).

Physiotherapy & Mobility Support – Assisting participants in following mobility plans and therapeutic exercises.

Occupational Therapy & Daily Living Skills – Supporting functional independence and the use of adaptive equipment.

Hospital Discharge & Transitional Support – Assisting with transition plans developed by health professionals.
